Responsibilities
Registered Nurse, Care Manager and Wellness Coach
Requisition Number 39137 Job Location Kansas City, MissouriJoin our team as a creative, clinically skilled professional! Provide hands-on care, triage, and wellness coaching while engaging patients through newsletters, webinars, and promotions. Enjoy a supportive environment with on-site amenities like a coffee shop, cafeteria, walking tracks, and pharmacy access.
Schedule
- 20 hours per week
- Fridays are required
- Additional days are flexible within Health Center operating hours. We are open Monday - Friday, 7:30am-5pm
What You’ll Do
- Develops personalized treatment plans using Lifestyle Medicine principles and member preferences
- Builds supportive relationships and serves as a trusted resource and advocate for members
- Assesses clinical, educational, and social needs using member input, data analysis, and medical records
- Provides self-care education and tools to support chronic condition management and behavior change
- Coaches members virtually via phone and video, delivering engaging, high-quality experiences
- Maintains patient health records to ensure accurate and up-to-date record
- Collaborates with care teams, vendors, and health center staff to coordinate care and wellness programs
- Identifies urgent situations and responds within legal scope of practice
- Promotes prevention-focused, multidisciplinary care that improves outcomes and lowers costs
- Participates in outreach, follow-ups, Care Team meetings, and other duties as assigned
- Other duties as assigned
What You’ll Bring
- Currently licensed Registered Nurse in state of practice
- Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
- Bachelor's degree (BSN) from four-year college or university preferred
- At least three years’ experience as a Registered Nurse caring for members with any of the following conditions: Type 2 diabetes, asthma, heart disease, heart failure or hypertension
- Experience in Condition Management, Certificate in Case Management, Care Coordination, Certified Diabetes Educator (CDE) Certified Diabetes Care and Education Specialist (CDCES) a plus
