Premise Health is proud to partner with Kwik Trip and are currently looking for a part-time Care Tech/Phlebotomist to join our team at our employer health clinic located in Appleton, WI!  

Schedule:     
 * Clinic Hours: Mon & Tue 7:00 am - 4:30 pm; Wed & Thu 9:00 am - 5:30 pm; Fri 8:00 am - 1:00 pm
 * Schedule to be built during clinic hours ~ 20 Hours per week ~ Pref Mon, Wed, Fri

About this Role:  The Care Tech/Phlebotomist will provide both administrative and clinical functions at our clinic where we provide care to patients ages 2+.  This will consist of front desk support, rooming patients, scheduling, vitals, and other duties as assigned.    

Essential Functions:

• Demonstrates the knowledge and skills necessary to collect blood samples from patients
• Works independently, in a safe and appropriate manner in accordance with
• safety and organizational policies and procedures
• Demonstrates problem solving prevention skills equal to the expected level of responsibility for the job
• Assists with supply inventory
• May instruct phlebotomy procedure to new employees
• Understands and abides by all of the BSFHC infection control guidelines and safety procedures; know location and use of MSDS material and use of emergency equipment
• This role has no supervisory responsibilities
• May require other duties as assigned.

Job Requirements:

• Current national certification (required for WA, CA and LA) in Phlebotomy from a Premise Health approved organization OR Certificate/Diploma and Transcripts from an accredited Phlebotomy program with hands on externship completed and passed- total online courses are not acceptable.
• CPR (BLS for community/lay responder or Healthcare Provider/Professional Rescuer - the copy must include the front and back of card as applicable) Certification must include “hands on” evaluation of skills by the trainer/instructor. Total on-line courses will not be accepted. Course must also meet the American Heart Association (AHA) standards but need not be an AHA certificate.
• State Phlebotomy Tech Certification (CA, WA and LA)

Preferred Experience:

• Six months to one year phlebotomy experience in a healthcare environment or satisfactory completion of an approved phlebotomy course and completion of 100 hours clinical experience
