We are looking for a Centralized Care Navigator to work remotely in support of our clients across the US. If you possess a “whatever it takes attitude” keep reading.

It’s hard to pinpoint a “typical” day here (and who wants typical anyway?), but the remote Care Navigator helps patients and their families traverse a complex healthcare system by interpreting and answering patient questions regarding care/benefit plans, health care management so patients can make informed decisions. The Navigator also conducts health center outreach.

What You'll Do For Us:

  • Care/Benefit Plans
    • Serves as the subject matter expert of clients’ benefits plan design and all partners that utilize it
    • Navigates client networks and providers and interpret information to patients and their families
    • Coordinates information flow of benefits between dental, medical and other healthcare providers within the medical community
    • Fosters communications and coordinates benefits with insurance companies with the goal of creating a smooth patient centered experience
    • Communicates and schedules appointments with specialists on behalf of members
  • Member Engagement
    • Conducts health center outreach
    • Collaborates with virtual team and onsite client teams to improve member engagement from goal planning through execution and after-action review.
  • Referral Management
    • Develop understanding of Epic's referral and scheduling capabilities, and available tools
    • Develop understanding of where member populations live and work, as well as high-value providers and facilities in key geographies
    • Proactively build relationships with community specialists, alongside other health center team members, to educate them on Premise and our expectations, facilitate streamlined referral access and information sharing
    • Interact with Premise providers to understand referral needs
    • Interact with members via multiple modalities (phone, email, video, etc) to identify appropriate referral target(s)
    • Work with community specialists and members to schedule appointments and ensure bi-directional notes transfer
    • Follow-up with member post-specialist visit, ensure member and specialist notes return to Premise, and coordinate ongoing care needs as appropriate
    • Remain in close coordination with central Connected Care+ team as we evolve our strategy, leveraging new tools and processes


  • Care/Benefit Plans and Member Engagement
    • Bachelor’s degree in a health-related field preferred, or equivalent experience.


  • Member Engagement
    • 3+ years working in corporate wellness/health promotion or with health system logistics/scheduling

    • Ability to handle sensitive information confidentially
    • Ability to closely adhere to policies, practices and procedures

    • Talking to others to convey information effectively
    • Ability to listen to and communicate information and ideas both verbally and in writing
